PCMH stands for patient centered medical home. Sometimes the synonymous term PCHCH is used which stands for patient centered health care home accreditation. PCHM is a patient centered model of care which places the patient at the center of all healthcare decisions and places the primary physician in charge of coordinating care, including specialists. The patient centered concept is gaining in popularity encouraging more and more practices to join the movement. Becoming a Medical Home provides a financial incentive for practices and therefore dangling another carrot to promote the model.
Beyond becoming a medical home is the process of accreditation. There are 4 national accreditation organizations which are responsible for medical home accreditation: NCQA, URAC, TJC, and AAAFC. Accreditation provides national recognition for achieving excellence in quality and patient care.
The public is very concerned and focused upon quality and is very astute in researching providers who meet nationally accepted standards of care. More and more data is becoming available providing the consumer with answers with the touch of a button. In response to the demands of the public, NCQA has developed the Consumer Assessment of Healthcare Providers and Systems (CAHPS) PCMH Survey. The CAHPS assesses the following areas of care:

- Information
- Communication
- Coordination of care
- Comprehensiveness
- Self-management support
- Shared decision making
The survey results indicate how well the practice is doing in terms of the Medical Home. The data is used to set benchmarks that allows consumers to compare different practices. The survey was created as a joint effort between NCQA and AHRQ. Practices who have achieved NCQA accreditation for their Medical Homes are eligible to apply for the additional distinction of CAHPS reporting. Practices must use an NCQA certified vendor for their data collection. The vendor must have meet the following requirements:
- Capture patient data via the CAHPS PCMH survey
- Use a standardized sampling process and attain the minimum number of complaint surveys
- Use an approved data collection
- Submit survey data to NCQA using a specified file layout and data submission method.
